Participant Waiver & Acknowledgment of Risk

Eligibility & Registration

All individuals must complete this waiver before taking part in any activity offered within a ROCKET ROOM venue ("Activities").

Participation in axe-throwing is strictly limited to persons 18 years of age or older. Minors may only engage in permitted activities if a parent or legal guardian completes this waiver on their behalf. Identification may be requested. No one may participate without submitting a completed waiver and meeting age requirements.

If you are signing on behalf of a participant under 18, you agree that you are releasing ROCKET ROOM from any and all liability connected to that minor's participation, including potential future claims brought by or on behalf of the minor.

Agreement to Follow Rules & Instructions

I confirm that I will comply with all written, visual, and verbal safety guidelines provided before or during the session. I agree to follow the directions of ROCKET ROOM staff at all times.

I understand that I am responsible for my personal safety, the safety of my belongings, and the safety of any minor for whom I am responsible while participating in Activities.

Health & Physical Condition

I certify that, to the best of my knowledge, I am physically able to take part in the Activities and do not have any health condition that could create an increased risk of injury to myself or others.

I agree to communicate any relevant medical or physical concerns to venue staff. I acknowledge that in some cases participation may not be permitted if it cannot be carried out safely.

If I am uncertain about my ability to participate safely, I understand that I must seek medical advice and refrain from participating.

Participant Acknowledgements

• I understand that participation may be refused if I appear intoxicated or behave in a way that could compromise safety.
• I acknowledge that consuming alcohol before or during axe-throwing is not allowed.
• I understand that ROCKET ROOM does not assess the abilities or limitations of individual participants and that I am solely responsible for determining whether the Activities are suitable for me.
• I understand that Activities may involve multiple groups and individuals of varying ages and skill levels.
• I recognize that axe-throwing and similar Activities carry inherent risks and may result in serious injury or death.
• I accept that participation involves physical exertion and requires an appropriate level of fitness.
• I acknowledge that I am accountable for my actions and decisions while using the facilities.
• I understand that ROCKET ROOM does not provide expert training, evaluation, or certification of participants' skills or abilities.

Assumption of Risk & Release of Liability

I understand that participation involves inherent risks & injuries may occur during normal use of the equipment and not solely as a result of defective equipment or organizational negligence.

Except where injury or death results directly from ROCKET ROOM's negligence, I agree that the Company is not responsible for any loss, damage, or injury connected to participation in the Activities or failure to follow safety rules. I waive any claim against ROCKET ROOM in relation to such matters.

Photography, Video & Personal Data

By entering the venue, I consent to ROCKET ROOM capturing photographs or recordings of me and using such media for promotional or informational purposes without compensation. I understand I may withdraw this consent by contacting ROCKET ROOM as outlined in its Privacy Policy.

I acknowledge that ROCKET ROOM will retain an electronic copy of this waiver and my personal information for three years and two months unless required otherwise by law. This is separate from any rights under data protection legislation.

Safety & Conduct Requirements

I confirm that:

• I am not under the influence of drugs or alcohol.
• I will comply with all posted signs, staff instructions, and safety rules.
• I understand that CCTV operates throughout the premises for safety and security purposes.
• I have been given access to, and understand, all relevant safety information.

Activity-Specific Rules: Axe Throwing

I agree to follow these rules at all times:

1. Only throw toward the designated target area.
2. Do not throw while any person is inside the axe-throwing lane or enclosure.
3. Do not enter the lane while another person is throwing.
4. Remain alert, behave responsibly, and avoid any action that could create a safety hazard.
